Add Active Problems #

For Snomed Code > Face Sheet > Problem History > Snomed Button

For ICD – 10 Code > Face Sheet > Problem History > DX Button

Example Task #

During your discussion, the patient informed you that he no longer has insomnia. Review and update the problem list.

Instructions #

For Snomed AND DX(ICD-10) Code:-

1. Select ADD at “Problem History” at the bottom – right hand side of the face sheet page.
   
   
2. Search for the Problem you are looking for. If your search returns no results, try searching using the first four letters of the problem name. Select the problem.
3. After search, you will get the Problems with snomed and icd-10 codes. Just select the button top – right hand side if you want to by snomed then select the button “SNOMED”  OR  “DX” you will get the Problem list with icd-10 codes.
   
   

Hot Codes:-

1. User can also able select the Problem History code in Hot Codes list.
2. Search for the Problem you are looking for. If your search returns no results, try searching using the first four letters of the problem name. Select the problem.
3. After Select Problem History code seen below left – hand side frame. Select the pencil sign as you want to add cede in “HOT CODES LIST”.
4. It will open one popup. Select “Add to my Hot Codes” with green check mark. Finally this code will add in Hot Codes list.

Modify Problem #

Face Sheet > Problem History

{ Onset Date, End Date, Active, Move to past, Delete this problem, Annotation } = the brackets mean options to select from.

Example Task #

During your discussion, the patient informed you that he no longer experiences headaches on the left side of his head but now he experiences them at the base of his skull. Review and update the problem list.

Instructions #

1. Select ADD under the “Problem History” at the bottom – right-hand side of the face sheet page.
   
   
2. Select the metrics you would like to change. Onset Date and End Date can be changed in particular text. You can provide instructions for the patient for the “Annotation”. You are also able to select “Active” or “Move to past” and delete also.
   
   

Display Active Problem #

For Snomed Code > Face Sheet > Problem History

You would like to review all studies you previously Active Problem list for this patient
